    The Occupational Therapist (OT) is responsible for the implementation of standards of care for occupational therapy services.  The incumbent is responsible for providing up-to-date, timely, and comprehensive occupational therapy plans of care to improve the quality of life of patients. OT will perform accurate, timely and measurable billing practices on a daily basis. OT will collaborate with other disciplines to ensure complete quality care of the patient and to enhance the health and overall quality of life through individualized therapy. OT will utilize knowledge of patient's age, background physical history and cultural diversity in the implementation of the patient care.


    ·        Evaluates patient's functional status (muscle function, endurance, visual coordination, written, verbal communication skills, self-care ability, and work capacity). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participate in the development of the total plan of care.

    ·        For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with agency policy.

    ·        Develops treatment programs and establishes goals for improved function.

    ·        Patient will be evaluated and plan of care established within 48 hours of referral. If unable to perform evaluation within 48 hours, physician will be notified.

    ·        May teach new skills or retrain patient once familiar daily activities that have been lost due to illness or injury.

    ·        Provides accurate and timely documentation consistent with the plan of care.

    ·        May train patient in the use of prosthetic and/or orthotic devices.

    ·        Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in patient's condition. Clinical documentation is due 24 hours after visit is provided.

    ·        Maintains contact/communication with other personnel involved in the patient's care.

    ·        Attends Care team conferences and in services, as required, by agency to ensure coordinated and comprehensive plans of care for the patient.

    ·       Participate in interdisciplinary patient care conferences each week and maintains ongoing communication with all disciplines involved in patient plan of ca re.

    ·        Identifies patient and family/caregiver needs for other home health services.

    ·        Instructs patient's family/care giver and other organization health care personnel in patient's treatment regime when indicated and appropriate.

    ·        When Occupational therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every (2) weeks.

    ·        Participates in discharge planning for patient.

    ·        Provides supervision and oversight to COTA’S.

    ·        Graduate of an accredited Occupational Therapy program

    ·        Registered with the National Registration Examination of the American Occupational Therapy Association.

    ·        Two years of Clinical experience as an occupational therapist.

    ·        Prior Home Care experience preferred.

    ·        Successful completion of Competency and Proficiency Skills evaluation appropriate for the position upon hire.

    ·        Possess and maintain current BLS CPR certification.

    ·        Demonstrates excellent verbal and written communicatio.

    ·        Basic computer skills and knowledge of Microsoft Office Products.

    ·        Licensed driver with automobile that is in good working order and insured in accordance with agency requirements.
