    Position Summary


    The Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.


    • Provides direct patient care as defined in the CA Nurse Practice Act and under the supervision of a Registered Nurse
    • Assists the physician and registered nurse in performing specialized procedures in accordance with physician orders and the plan of care
    • Provides accurate and timely documentation of clinical/progress notes consistent with the plan of Ensures that documentation is completed in the Home Care Home Base system within the time parameters established by the agency
    • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
    • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the registered nurse and/or Clinical Coordinator, Director
    • Participate in interdisciplinary patient care conferences each week and maintains ongoing communication with all disciplines involved in patient plan of ca re
    • Prepares equipment and materials for treatments observing aseptic techniques as required
    • Uses equipment and supplies effectively and efficiently
    • Assists the patient in learning appropriate self-care techniques
    • Participates in personal and professional growth and development
    • Performs other duties as assigned by the registered nurse

    • Graduate of an accredited practical nurse or vocational nursing
    • Preferred three (3) years nursing experience. Community/home health or medical/surgical experience is preferred
    • Currently licensed as a licensed practical nurse or licensed vocational nurse in the agency state of operation
    • Complies with accepted professional standards and
    • Demonstrates excellent verbal and written communication
    • Licensed driver with automobile that is in good working order and insured in accordance with agency
    • Basic computer skills and knowledge of Microsoft Office Products
    • Ability to develop demonstrated proficiency in utilization of the Home Care Home Base health record system
